Sarah Hyland, the Modern Family actress, has been granted a temporary restraining order against ex-boyfriend Matthew Prokop, TMZ reports. According to the gossip site, Hyland claims Prokop, whom she dated for five years, chocked and abused her during their relationship.

Us Weekly confirmed through Hyland's lawyer, Lee A. Sherman, that  "on September 19, 2014, Ms. Hyland obtained a Domestic Violence Temporary Restraining Order against Matthew Prokop" and that the "documents filed speak for themselves."

According to the document obtained by TMZ, Prokop allegedly pinned Hyland down against her car during a dispute about her attire, yelling "c-nt, c-nt" while choking her.

"His grip was so tight that I could not breathe or speak. I was scared and in fear for my life," Hyland, 23, explains in the documents. She asked her Modern Family co-star and TV mom, Julie Bowen, to help mediate and end to her relationship with Prokop. After Hyland purchased him a plane ticket back to Texas, it ended with him feeling threatened when he saw Bowen and immediately "ran outside into the backyard and began screaming." Bowen apparently stepped in and told Hyland to leave the house because it wasn't safe.

Threats were not only made to Hyland but also her pet dog and property, "[He] relentlessly bombarded me with vile, threatening and emotionally disturbing texts and voice mails including his own suicide threats," Hyland explains.

According to Hyland, Prokop entered a rehab facility on Sept. 21 but was released shortly after. A Los Angeles judge granted Hyland the retraining order as the rehab facility's director believed it was in her best interest due to his mental state.

Prokop is now required to stay at least 100 yards away from Hyland and her dog at all times.
